POSITION SUMMARY
The Perinatal Program Manager is responsible for the administration, coordination and management of day to day operations for CPSP and Care Coordination activities at the Perinatal Department’s Oakland site. The incumbent is responsible for program development of the full array of Perinatal services needed for prenatal care coordination which will support attainment of high-quality perinatal care for pregnant and postpartum women including; improved community access, system navigation and sustainability. This role will participate in the strategic development of goals and sustainability plans within the Medical, Community Wellness, and Integrated Behavioral Health departments. The Perinatal Program Manager will manage the development of new project areas that align with these goals and maintain a modest patient case load, ensuring compliance with state and federal regulations, and implementing strategies to improve perinatal health outcomes. This role also has Project Direction responsibilities for the Path Cited III Grant.
The Native American Health Center is an accredited institution and adheres to the standards of excellence set forth by the Accreditation Association of Ambulatory Health Care (AAAHC) and the Commission of Dental Accreditation (CODA).
DUTIES AND RESPONSIBILITIES
1. Manage planning activities with Perinatal staff through regular communications with this team through telephone calls, e-mails, meetings, trainings, webinars and regular written reports.
2. Provide training, support and performance evaluations to ensure team members meet program standards and create a collaborative and positive work environment.
3. .25 FTE status will be spent providing direct, patient-facing CPSP services.
4. Program Management which includes oversight of the daily operations of perinatal care coordination ensuring program goals and objectives are met.
5. Address member concerns and resolve issues promptly and effectively while promoting health education and resources to members and their families.
6. Develop and implement policies and procedures to enhance program effectiveness and efficiency.
7. Manage the alignment of program development and implementation with Perinatal strategies, including strategic planning, meeting facilitation, training, evaluation, and fund development.
8. Provide project direction for the Path Cited Grant planning grant, including budget oversight, implementation reporting, evaluation, research design, and sustainability.
9. Identify and ensure the incorporation of culturally appropriate prevention practices.
10. Work in concert with the Data Analyst and Program Evaluator to ensure that all required data strategies, collection, and documentation of program objectives and outcomes occurs. Utilize this data as a tool to monitor program performance, and drive improvements when necessary.
11. Provide monthly, quarterly and other reports required by the department and specific projects.
12. Assist with overall Department growth responsibilities, such as grant writing for this program area and participating in relevant committees.
13. Conduct community meetings, workshops and trainings as required and ensure that these meetings operate in a timely and effective manner.
14. Coordinate activities with partner agencies involved in federal, state and local projects, including developing and reviewing quarterly reports to funding agencies.
15. Resolve staffing issues and facilitate conflict resolution for staff under direct supervision. Hire, discipline, evaluate and terminate staff in coordination with the Human Resources department.
16. Actively participate in internal quality improvement teams and work with members proactively to drive quality improvement initiatives in accordance with
the mission and strategic goals of the organization, federal and state laws and regulations, and accreditation standards, when assigned.
17. Proactively plan for audits, site visits, and reporting. This includes, but not limited to developing and/or reviewing audit agenda and materials, collecting necessary documents and collating information in a presentable manner.
18. Patient Care: Demonstrate understanding and apply working knowledge of safety policies and ensuring safe patient practices.
19. Employee Safety: Safely performs all duties; follows required protective protocols to ensure personal safety as well the safety of others.
20. Must maintain compliance with ergonomic safety standards; be mindful of posture and regularly practice ergonomic stretches.
21. Work extremely well under pressure including meeting multiple and often competing deadlines.
22. At all times demonstrate cooperative behavior with supervisors, colleagues, clients and the community.
23. Other duties as assigned by supervisor.
MINIMUM QUALIFICATIONS
1. Master’s Degree major in related field OR Bachelor’s Degree with five-years’ experience working in maternal health, women’s health, comprehensive perinatal support program (CPSP), or other perinatal community health programs.
2. Previous supervisory or management experience.
3. Intimate knowledge of pregnancy, postpartum, referral to health resources in Alameda County.
4. Comprehensive Perinatal Services Program (CPSP) certified/eligible
5. Familiarity with the San Francisco Bay Area American Indian Community and Native-serving organizations.
6. Experience with strategic planning processes.
7. Proficient in data analysis, report writing, patient charting, and knowledgeable in CPSP guidelines and state/federal regulations.
8. Excellent social, writing and interpersonal skills.
9. Expertise with Windows, Microsoft Office, Excel and Word.
